One thing is clear it’s the same pen, the same manufacturer, but depending on where you buy it, you could pay anywhere from £80 to £100 more or less per month.

Price by Dose Full Breakdown (2.5mg to 15mg)

Treatment always begins at 2.5mg and the dose is increased every four weeks as your body responds. Each pen contains four weekly doses, meaning one pen lasts one full month. Here are the April 2026 prices:

Why Do Prices Vary?

There are two main reasons prices differ so widely. First, in September 2025 Eli Lilly raised wholesale prices and some pharmacies passed this cost on to patients while others absorbed it. Second, some pharmacies charge separately for consultation, needles, sharps bins, and delivery, while others include everything in a single price. Specialist online pharmacies are often cheaper than high street chains due to lower overheads but don’t make your decision based on price alone. Quality of service matters just as much.

September 2025 Price Increase What Happened?

If you’re wondering why Mounjaro wasn’t this expensive before, understanding this price increase is important. The increase came directly from Eli Lilly and fundamentally changed the private pricing landscape for Mounjaro across the UK.

Why Did Prices Go Up?

On 14 August 2025, Eli Lilly announced that UK wholesale prices for Mounjaro would increase by up to 170% from 1 September 2025. The highest dose 15mg went from £122 to £330 per pen. The 2.5mg starter dose rose from £92 to £133. The company stated that the UK launch price had been deliberately set well below the European average to help accelerate NHS access. They are now aligning UK pricing with other developed countries. The timing also coincided with US administration pressure on pharmaceutical pricing, though Lilly has not officially confirmed a direct connection.

One important detail Lilly also offered pharmacies a rebate programme. Pharmacies that sign this commercial agreement can claim back a portion of the cost and therefore offer patients a lower price. This is the single biggest reason why prices vary so widely across the UK for the exact same pen.

Did NHS Prices Change?

Not at all. NHS prices were completely unaffected by this increase. Eli Lilly has a separate agreement with the NHS to keep supply and pricing stable. In England, NHS patients still pay only the standard £9.90 prescription charge. In Scotland and Wales, it is entirely free.

NHS vs Private Mounjaro

There are two routes to accessing Mounjaro in the UK and the difference in cost, eligibility, and waiting time between them is enormous.

NHS Eligibility Who Qualifies?

Here is something critically important to understand clearly. The NHS rollout has expanded in 2026, but eligibility remains limited to high-risk patients only. Many people hear that Mounjaro is “available on the NHS” and assume they can get it but the reality is that the majority of people will still need to go private.

To access Mounjaro through the NHS, you must meet all of the following criteria simultaneously. Your BMI must be 40 or above or 37.5 if you are of South Asian, Black African, or Chinese background. You must also have at least four serious health conditions such as Type 2 diabetes, high blood pressure, heart disease, or sleep apnoea. You cannot self-refer eligible individuals are identified through GP records and contacted directly.
